In the analytical phase, errors within this phase are met with a wide range of frequencies from 4% to 42.1% and the most common form of error relates to the inaccuracy of interpretation with serious consequence on both patient and pathologist (Mariana et al., 2018).

In the post-analytical phase, errors falling in this phase are less prevalent than those in the preceding phase, with a rate up to 8%. The commonly encountered errors in this phase are the following: wrong validation of data, a delay in TAT, and improper data entry (Mariana et al., 2018).
