Obesity is a well-established risk factor for cardiovascular disease (CVD). It is linked to a higher risk of CVD, notably heart failure and coronary heart disease. Changes in body composition can affect hemodynamics and alter heart anatomy, which are two processes through which obesity increases CVD risk. According to the paper, recent studies show that adipose tissue (AT) is an organ that releases substances that can affect the cardiovascular system, either in an endocrine or paracrine way. Under a physiological setting, AT can control normal cardiovascular function, but it can also undergo a phenotypic shift, which leads to the development of pro-inflammatory properties. However, AT depots can change their phenotype in response to local cues from the vascular and myocardial systems. Taking note of this ability of the AT depots could lead to the development of new CVD diagnostic and predictive tools. I think it is interesting that the paper has shown that obesity may have a protective role in chronic diseases such as CVD. Further study into the endocrine and paracrine bi-directional loop between the cardiovascular system and AT would be very promising, as it can help in enhancing future clinical practice and in identifying new therapeutic targets.
