The article, authored by E.K. Oikonomou and C. Antoniades, focuses on accumulating knowledge on the biology and function of adipose tissue has caused a significant shift in our understanding of its role in health and illness. In this article, I found out that adipose tissue is increasingly recognized as a major regulator of cardiovascular health, mediated by the secretion of numerous bioactive molecules, including adipocytokines, microvesicles, and gaseous messengers, with a wide spectrum of endocrine and paracrine effects on the circulatory system. The function and secretome of adipose tissue are closely regulated by complex homeostatic mechanisms and local cell-cell interactions, which can become dysregulated in obesity. The adipose tissue secretome changes from anti-inflammatory and anti-atherogenic to pro-inflammatory and pro-atherogenic in response to systemic or local inflammation and insulin resistance. Moreover, the interaction between adipose tissue and the circulatory system is bidirectional, with vascular- and heart-derived signals directly influencing adipose tissue function. The article synthesizes the current understanding of the biology and geographic variability of adipose tissue in humans, elucidating the complex molecular mechanisms directing the crosstalk between adipose tissue and the circulatory system, and their potential therapeutic application. Lastly, the authors address how therapeutic targeting of adipose tissue can improve cardiovascular disease prevention and treatment.
